Taking a small number of design partners for paid pilots this quarter. Talk to us

For care managers and home-care teams

A new client, set up in ten minutes.

Not two hours. Photograph the discharge packet — OurCareBook reads the medications, the follow-ups and the warnings, and your coordinator confirms them. The family sees the same plan, at no extra cost.

Starts with a short paid pilot and one agreed measure. No migration required.

One whole workflow, start to finish — for each way people use it.

Why it helps: Intake drops from about two hours to ten minutes, and families stop ringing the office to ask how the visit went.

    Create an account

Three ways in. Pick the one that sounds like you.

Nobody has to know what “B2B” means. You say who you are, and you land on the shortest path for that person — then join other teams or families whenever you like.

    Everything we support

B2BAgencies, care managers, clinics and nonprofits add clients, office staff and field caregivers — each with the right access.

B2CFamilies run a free care book: medicines, appointments, paperwork, and every sibling on the same page.

C2CFamilies hire an aide directly. The aide logs visits and incidents; legal and money papers stay private.

    AI agents

The agents do the reading, checking and writing. People make the calls.

Watch one work. It reads, reasons out loud, corrects itself, and stops to ask a person when it isn't sure — then hands back hours that used to go on typing, chasing and phone calls.

Illustrative example of one team's week — not a customer result. Your numbers come from your own Reports.

Six agents, one rule: a person decides.

They only see what that person could see, and they never give medical advice.

    • Intake agent

Turns photos of discharge papers, prescriptions and pill bottles into medicines, appointments and warnings — each beside its source line.

    • Care-check agent

Finds the follow-up nobody booked, the refill running out, the missed doses and the family that's heard nothing.

    • Triage agent

Ranks the whole caseload by what will go wrong next, and says why in words.

    • Family-update agent

Writes the weekly note to the family from visits, doses and appointments.

    • Ask the care book

Answers any question about a client from their record alone, citing where it found it.

    • Safety checker

Flags allergy clashes and duplicate medicines the moment they're added.

    The platform

Everything after intake, in one record.

A caseload that tells you who to ring first

Clients ranked by what will go wrong next — overdue work, unrecorded doses, a family gone quiet for nine days. Each one says why in words, because a score with no reason attached stops being trusted by week two.

One care plan the whole circle can see

Medications, appointments, documents and contacts in a single record. Your coordinator, the family and the hired help each see the part that is theirs — a nurse gets the medication list, not the will.

A family portal that costs you nothing

Families are included in every subscription. They read updates, tick off doses and arrange lifts — fewer calls into your office, and a record of care that speaks for itself at review time.

Reports that answer the board's question

Median setup time, task closure and activity per client, exportable and printable, with a full audit trail of who saw what and when.

Caregiver visits the family can actually see

Aides clock in, tick the care plan, note mood, meals and vitals, and clock out. The visit lands in the family's updates the moment it ends — the call your office doesn't have to take.

    Incidents that can't get lost

A fall or a medicine error goes to the top of the caseload with its severity and what was done, and stays there until a named person follows it up.

    Why teams switch

Everything a small team needs. Nothing it has to wait months for.

Enterprise home-care suites are built for scheduling, EVV and billing at scale. OurCareBook is the care record everyone shares — and works alongside the scheduling or billing tool you already have.

    Who it's for

    Small teams carrying real caseloads.

Built for the organisations large platforms treat as an afterthought — where the person who buys it is usually the person who uses it.

    Geriatric care managers

Independent and small practices carrying 15–80 clients.

    Dementia navigation teams

Including programmes standing up under CMS GUIDE.

    Home-care agencies

Non-medical agencies with 25–250 active clients.

    Senior-service nonprofits

Area Agency on Aging contractors and community programmes.

    Security

Built to hold a medical record.

Access is enforced in the database, not only in the screens. A bug in a page cannot show somebody a record they have no right to see.

Ask us directly what is signed. We will tell you.
    • Row-level security

Every table. Access decided by membership of the client's circle, in the database itself.

    • Per-category visibility

Legal, insurance and financial paperwork restricted by default.

    • Full audit trail

Who looked, who changed, and when.

    • Human confirmation

No extraction becomes a record without a named person approving it.

    • Private documents

Short-lived signed links. Nothing is ever served publicly.

    • Export and deletion

Take a copy of everything, or ask for all of it to be erased.

    Pricing

Priced for small teams, and published.

No "request a quote" for a two-person practice. Families are always free.

    Compare plans

Most teams start with an 8-week paid pilot at $1,500, credited to the first year if they continue.

    Questions

    The ones worth asking first.

    • No. It transcribes; it does not practise medicine. Everything it reads comes back as a proposal shown beside its source line, and a named person confirms each one before it becomes part of the record — their name is stored against it. It never infers a dose that is not printed, and it flags what it could not read rather than guessing.
